Medical Home Learning Collaborative 2
HomeOverview
Home
The Medical Home Learning Collaborative has dual-purposes: to improve the quality of care for children with special health care needs and their families by implementing the medical home model in primary care practices and to build capacity in Title V agencies to sustain and spread the Medical Home model in primary care practices in their states.

Success Stories

"Our approach to families of CYSCHN has changed from reactive to proactive-we believe our quality of care has improved as a result of participating in the MHLC 1, and we are much more in touch with local community of services. We are more organized, and now are seeing improvements in efficiency as well."                         --Robert Terashima, West Jordan, Utah

"The participation benefits of the Ohio Title V CYSHCN Program in the first MHLC were many. It allowed us to concentrate on the development of the Medical Home in a very concrete way. The MHLC has made the rapid learning cycle a part of what we do. We look for small success upon which we can build. Our participation had raised the consciousness of the Medical Home throughout Ohio."                                            --Dr. Bryant, Columbus, Ohio
