ADHD

The ADHD learning collaborative used the Care Model for Child Health as the framework for translating the recommendations of the AAP's Practice Guidelines into practical steps for primary care practices.  The change package lists changes in seven areas:

Clinical Information System--with specifications for a registry

Decision Support--including strategies for partnership with mental health expertise

Delivery System Design--with a focus on planned visits

Family Management Support--including recommendations for behavioral strategies and care plans.

Self Management Support --including engaging teens increasingly in managing their own care.

Community Resources--recommending partnerships with schools and help families understand their rights in the school setting.

Health Care Organization--including recommendations for benefit design and financial incentives
