Asthma

NICHQ uses the Care Model for Child Health as its framework for translating the recommendations of the National Heart, Lung and Blood Institute Practice Guidelines (also known as Expert Panel Recommendations II) into practical steps for primary care practices.  The change package lists changes in seven areas:

Clinical Information System--includes specifications for a registry

Decision Support--includes recommendations for routine severity assessment and use of controller medications for patients with persistent disease.

Delivery System Design--focuses on the importance of planned visits

Family Management Support--includes recommendations for asthma action plans and shared goal setting.

Self Management Support --emphasizes engaging youth in self assessment and management as is developmentally appropriate

Community Resources--recommends partnerships with schools, coaches, and after school programs, and the use of culturally appropriate tools and materials.

Health Care Organization--provides recommendations for benefit design and financial incentives
