Process Measure: Follow up - Healthy Care for Healthy Kids Learning Collaborative*
Definition
Percentage of children, 2 to 12 years of age, with follow-up planned within one month after classification for overweight
Numerator: Number of children, 2 to 12 years of age, with a follow up planned a month after classification of being overweight in measurement period month
Denominator: Number of children (or charts reviewed of children), 2 to 12 years of age who are identified as overweight in the measurement period month
Goal
95 or greater percentage of children, 2 to 12 years of age, with follow-up planned within one month after classification for overweight
Data Collection Plan
All children, 2 to 12 years of age, should be checked for the need for a follow-up.
