Definition
Percentage of children, 2 to 12 years of age, who are classified as overweight or at risk for overweight who have a management plan and goal documented in medical record
Numerator: Number of children, 2 to 12 years of age, who are classified as at risk for overweight or overweight, who have a management plan and goal documented in medical record either at a well child visit
Denominator: Number of children (or charts reviewed) of children 2 to 12 years of age, who are classified as at risk or overweight who had well child visit in measurement period month
Goal
95% or greater percentage of children, 2 to 12 years of age, who are classified as overweight or at risk for overweight who have a managment plan and goal documented in a medical record
Data Collection Plan
All children 2 to 12 years of age should be checked at visits
