Overweight Children - Healthy Care for Healthy Kids Learning Collaborative*

Definition

Percentage of children, 2 to 12 years of age, with BMI percentile equal to or greater than 95th percentile for age and gender

Numerator: Number of children, 2 to 12 years of age, with BMI equal to or greater than the 95th percentile for age and gender for measurement period month

Denominator:  Number of children 2 to 12 years of age, with a well child visit in the measurement period month

Note: If you use a registry, the numerator is the number of registry entries, with a well-child visit in measurement period month, whose BMI is at or above 95th percentile for age and gender.  The denominator is the total number of children with a well-child visit in measurement period month

If you collect data using a chart review then audit 30 well child visits in the measurement period month.  Select charts using a convenience sample.

If you design a flow sheet for data collection and collect data on all well child visit, then the denominator will be the number of flow sheets collected each month


Goal

Reduce the percentage of children, 2 to 12 years of age, with BMI percentile equal to or greater than the 95th percentile for age and gender

 

Note: the rate may increase as you begin the project, then stabilize as identification interventions are put into place; this should be followed by a reduction over time


Data Collection Plan

Each month, collect data by using chart audit, or by entering all overweight and at risk for overweight children in a registry.  You may design a flow sheet if you want to collect data on all encounters

Each measure will require that you enter the numerator and denominator regardless of data collection method used
