Definition
Patient and family satisfaction with practice related to support provided for children who are at risk or overweight.
Include two questions below:
1. A personal doctor is the doctor that you usually call if you need a check-up, if you get sick or hurt, or if you want advice about a health problem.
Using any number from 0 to 10, where 0 is the worst personal doctor possible and 10 is the best personal doctor possible, what number would you use to rate your personal doctor?
2. Would you recommend your doctor to friends/family? ( Yes, definitely, probably, Not sure, Probably not)
Goal
Improve patient and family satisfaction with practice related to support provided for children who are at risk or overweight.
Data Collection Plan
Query 20 parents of children who are at risk for overweight or are overweight. Please include these 2 questions. You could include questions about other areas of focus (care planning, care partnership support, etc.) You may add these two questions to an instrument you already use.
The answer to question one is entered as an average. Sum up the total scores and divide by the 20 responses to get the average score.
The answer to question two is the percentage of responders who selected “Yes, definitely"
