Healthy Care for Healthy Kids Toolkit* - Community Resources Tools

NICHQ, the National Initiative for Children’s Healthcare Quality, and the teams from NICHQ’s Learning Collaborative Healthy Care for Healthy Kids, a Collaborative to Prevent, Identify and Manage Childhood Overweight, sponsored by Blue Cross Blue Shield MA, have developed a toolkit to assist clinicians in providing quality care for children who are at risk for overweight, or overweight. This toolkit provides basics tools for primary care practice teams to deliver coordinated, integrated, and multidisciplinary services to both prevent overweight and improve care for children who are already overweight or at risk for overweight.

 

Physician Involvement in Community Action

The authority vested in physicians gives your voice clout on the issue of childhood obesity prevention and treatment. It is not necessary for you to create your own community programs and projects. Some of the most efficient and effective ways to turn your authority into action are listed on this worksheet.

 

Community Resources Key Points

In NICHQ’s Care Model for Child Health (based on the Chronic Illness Care Model developed by Ed Wagner), the health care system can optimize self-management support by informed, activated patients through collaboration with community resources.
