The registry helps practices/clinics identify and manage patients who are at risk for overweight or overweight. It can also help idenify preventive measures the practice/clinic can take in the 0- 2 year old population.


Directions

Instructions for use:

• Select sheets using the tabs at the bottom of the Excel window

• Use drop-down controls on the sheets to select clinics and measures.

• Always enter a patient's demographic information on the Patient List before entering encounters on Encounters

• To add a new encounter for an existing patient, go to the Encounters sheet, enter the encounter date in the next available blank row, and select the patient's name from the drop down list. 

• Age and BMI statistics will recalculate as you enter necessary data

• Patient List is formatted for printing

• Remember to save the worksheet each time you add new data

• IMPORTANT: DO NOT LEAVE BLANK ROWS IN THE PATIENT LIST OR ENCOUNTERS (they will spoil the calcluations!)
