NICHQ, the National Initiative for Children’s Healthcare Quality, and the teams from NICHQ’s Learning Collaborative Healthy Care for Healthy Kids, a Collaborative to Prevent, Identify and Manage Childhood Overweight, sponsored by Blue Cross Blue Shield MA, have developed a toolkit to assist clinicians in providing quality care for children who are at risk for overweight, or overweight. This toolkit provides basics tools for primary care practice teams to deliver coordinated, integrated, and multidisciplinary services to both prevent overweight and improve care for children who are already overweight or at risk for overweight.
Body Mass Index Chart for Boys age 2-20
This chart can be used for individual male patients to track BMI percentiles. It includes the formula for calculating BMI as well as chart to plot BMI by age visually.

This chart can be used for individual female patients to track BMI percentiles. It includes the formula for calculating BMI as well as chart to plot BMI by age visually.
Approach for Prevention & Management of Overweight in Children 2-12 years
This algorithm, developed as part of the NICHQ and Blue Cross Blue Shield of MA Healthy Care for Healthy Kids Learning Collaborative, provides a succinct, pictorial approach to preventing and managing care for overweight children.

These charts determine systolic and diastolic blood pressure by height percentile from a standard growth curve.
