Next Steps: A Practitioner's Guide For Themed Follow-up Visits For Their Patients to Achieve a Healthy Weight

The all-new Next Steps is a guide with flip chart for clinicians to help children and adolescents reduce and manage weight. The guide outlines a series of planned follow-up visits designed to teach evidence-based weight management strategies that can lead to success for the practitioner and patient. The detailed flip chart is to be reviewed by the practitioner and family. Next Steps was created through a partnership among the National Initiative for Children's Healthcare Quality, The Barbara Bush Children's Hospital at Maine Medical Center, and the Maine Chapter of the American Academy of Pediatrics. It is available for purchase on the AAP website.

The Healthy Weight Collaborative Action Kit is designed to help spread the lessons learned from the Healthy Weight Collaborative to those interested in making change in their communities.

Healthy Care for Healthy Kids

NICHQ, along with teams from Healthy Care for Healthy Kids, a Collaborative to Prevent, Identify and Manage Childhood Overweight, sponsored by Blue Cross Blue Shield MA, have developed a toolkit to assist primary care practice teams in providing coordinated, integrated, and multidisciplinary services to both prevent obesity and improve care for children who are already overweight or at risk for becoming overweight.
